Crafted items being duplicated

I have a problem on my server. all that one craft is done in duplicate

how to fix this problem

 

Do you have CraftingController, or other plugin that can modify crafting?

I same problem I'm not CraftingController
I just use Instant Craft

unload each crafting plugin one at a time then test until you find the buggy plugin
