Unauthorized players getting admin?

I have had an odd problem lately. Just today I've noticed 2 players had admin permissions and were TPing to players removing bases etc. I'm confused as to how they are making themselves admjn because I have very little admins on my servers and I know the ones I do. I am the only one with rcon access and ownerid tag. I use the plugin fauxadmin for all my other admins so no one else can add anyone. Any ideas?

 

Maybe your rcon password is bad or the server is not well protected and got hacked.

 

Check to make sure your admins don't have infected binds, I've noticed a few popping up recently. Usually bounded to mousewheel, something like:
