Everything you destroy duplicates

Hi i updated my server to 3.4.2 version today and everything that you destroy is duplicating, for example if theres's a chest with c4 in it and you destroy chest with hammer it stays with items and the little chest is dropping with all the items.. any fix for that? please need help asap as players are complaining
